I didn't see anything about "All Souls Day" in the Book of Common Prayer, just All Saints Day, but did find this in the Canadian Book of Alternative Services.

Father of all,
we pray to you for those we love, but see no longer.
Grant them your peace,
let light perpetual shine upon them,
and in your loving wisdom and almighty power,
work in them the good purpose of your perfect will;
through Jesus Christ our Lord,
who lives and reigns with you and the Holy Spirit,
one God, now and for ever.

Anyone know why this day was added to the Anglican Calendar?

All Souls' Day is a Roman Catholic holy day. I would guess it ultimately originates in pagan ancestor worship.

According to Wikipedia, in the Church of England it is optional, and is described as a "comemmoration of the departed".

Obviously, the prayer given above presents some serious problems.
 
There is definitely a bit of Anglo-Catholicism in that prayer book above.

Creepy, yes, and idolatrous as well.

Augustine (354-430): For prayer is not righteous except through Christ, whom he sold in his atrocious sin: but the prayer which is not made through Christ, not only cannot blot out sin, but is itself turned into sin. NPNF1: Vol. VIII, Exposition on the Book Psalms, Psalm 109, §9.
