Although they are brief treatments, both Reformed and Evangelical Across Four Centuries by Feldmeth, et al. and Studies in Southern Presbyterian Theology by Morton H. Smith do well to present the basics of the issues at hand. The latter deals more with confessional subscription in the history of the Presbyterian church (primarily in the South).
