I need protocol help

Perhaps someone on the otherwise of the pond can help.

Yesterday I got an email from Lady Elizabeth Catherwood with some questions about her Father's sermons in Pensacola in 1969. (I was there, and was able to provide recordings to the MLJ Trust last year.)

What is the proper address to use (Lady Elizabeth, Lady Catherwood, or Lady Elizabeth Catherwood)? I doubt that simply Elizabeth would be proper.
 
I do believe the correct way is either My Lady or else Lady Catherwood

Actually I have just been informed My Lady should only be used when you are talking with her. If you write then the correct format is Dear Lady Catherwood
