Accountability among the Elders

I would like to hear in which ways the Elders at your local church keep each other accountable, or any good principles that you can share here about such accountability. The context is specifically among the elders and within the local church, without external oversight (I’m a 1689 Baptist, but I welcome all insights).

As elders we are accountable to each other in every way. We have the prerogative to ask questions and confront sin when necessary. Elders can bring discipline before the church of a fellow elder. We also try to defer to one another even in situations that are not "confrontational." One of the main purposes of plurality is mutual accountability, and we take that seriously. However, this happens naturally. In other words, we don't have meeting time set aside to ask, "How are you doing with ___ "? People can lie for those questions and fake things anyway.

Thank you for the response. How does counselling work, in your setup? If one elder counsels a congregant, how is this discussed with the others?
