What are the best domain providers to purchase a URL?

Also, what about for site templates?

I use porkbun.com. I have found them to be affordable and also provide the unusual top level domains I sometimes need to register, like .bible

This will depend on your CMS (Wordpress, etc). If you are like me and prefer to hand write HTML, I use the templates on html5up.net

We use Hover for domain. In the rare event that we need to reconnect (after rebuilds) it seems we have far fewer issues than what others experience in getting search engines to find us.

We've been on Squarespace forever. You can start with one of their templates or build from scratch. A ton of third party providers build templates for Squarespace too. Be sure to use version 7.1. Also, think through what you are trying to achieve before looking at templates. That will drive your color and font choices.

Domains, lots of good providers but I like NameSilo.

Site templates depend on what content management system you are using. Squarespace is great for easy to design sites. WordPress is better if you need more features that SS doesn’t offer but you will need a web host - SS is a content management system and web host rolled into one.
