Charles Hodge, Discussions in Church Polity (1878; rpt., Scarsdale, NY: Westminster Publishing House, 2001). Hodge treats both the principles and applications of church polity.

Off the top of my head here:

From a Baptist perspective, I would recommend John L. Dagg's Manual of Church Order, which is a classic. Gano republished it in 1990.

Although I have a passing familiarity with some of the documents therein, (many of them are available on Google Books) I don't have Mark Dever's Polity but I would expect it to be an excellent resource. It is also available online. In general IX Marks material will be excellent from a elder led congregational perspective.

Don Elbourne has also posted some historic Southern Baptist material.

The Madison Avenue Lectures is an imporant 19th Century Northern Baptist work. There are a lot of other historic works on Google Books or elsewhere online.

MacArthur's Master's Plan for the Church is well worth a look too, perhaps especially on the work of elders.
